     2. Birth certificate of the child (children)

     3. A certificate from a pediatrician that a healthy child was born naturally by a surrogate mother.

     4. Marriage certificate (if the surrogate mother is married).

     5.     Certificate of dissolution of marriage

     6. Application- consent of the spouse to the use of assisted reproductive methods and technologies and the conclusion of a surrogacy contract
